Travel Documents for UH-Based ADAP employees In order to facilitate booking a reservation, please fill out the ADAP Travel Request form at least 3 weeks before the anticipated departure date. Once the travel reservations are booked, the traveler will receive a travel packet with the flight and hotel reservations, the travel advance (if applicable), and the ADAP Travel Completion paperwork. When travel is complete, it is the responsibility of the traveler to complete the following forms: Flight Form, Itemized Expense Form, and the Trip Impact Form.
Flight Form: Please enter the actual flight arrival and departure times as this may effect the amount of travel per diem received.
Itemized Expenses Form: Please enter the extra expenses incurred during travel. Reimbursable expenses includes taxi fares to/from the airport or meeting, internet access fees for checking office email accounts, and airport departure fees.
Trip Impact Form: Please provide a description of the activities conducted during your travel, and the expected impact of these activities. |
